Health Care Program for Children with

Special Health Care Needs (HCP)        


 

HCP provides specialty medical clinics, care coordination and information for children with special medical care needs and their families.  The HCP staff assists families to coordinate care with other local agencies that serve children.  The HCP program is administered through the Colorado Department of Public Health and Environment.  Each year, the Delta County HCP program receives a limited amount of money to arrange two local pediatric orthopedic clinics and three pediatric neurology clinics.

 

We believe all children with special health care needs should be given the opportunity to obtain services which facilitate their optimum development.  Children with special health care needs are children first and must not be identified by their disability alone.  Parents should be provided with the necessary assistance in strengthening family life and in coping with the unique needs of their children.  Children with special health care needs and their families are entitled to the services of skilled qualified professionals who have experience and expertise in the care of children with special developmental and handicapping conditions.

Who qualifies for HCP services?

Children

  • whose families live in Colorado;
  • who are under 21 years of age;
  • who have a special HCP-eligible medical condition or diagnosis; and
  • who are referred by a physician or other professional.

 

What services can HCP offer for your child?

  • Access to local specialty clinics;
  • A local Nurse Coordinator to help locate and establish needed services for children with complex medical needs; and
  • Information to families about health care issues and the child's condition.

            Bicycle Helmets

Bicycle Helmets can be purchased at cost at the Health Services building, 255 West 6th Street in Delta.

 

            Infant Car Seats

Infant Car Seats and Booster Seats installation inspections can be arranged in cooperation with the Delta Police Department.
