Delta County Health and Human Services also serves as a three county district Single Entry Point for Options for Long Term Care. In Delta County, our offices are located at the North Fork Annex in Hotchkiss. This Agency serves as gatekeeper to long-term care programs funded under Medicaid. Individuals within the county may obtain information and guidelines to available long-term programs by calling the office at 872-1000 and asking to speak with a case manager on site.
Individuals in Gunnison and Hinsdale counties may reach the satellite office in Gunnison by calling 970-641-7984, or the Hotchkiss office by calling 1-800-439-5206.
