HCP provides care coordination, information, and referrals to regional specialty medical clinics for families and children who have special medical care needs. The HCP staff assists families to coordinate care with other local agencies that serve children. The HCP program is administered through the
Colorado Department of Public Health and Environment.
Working from a "Medical Home Approach," which identifies the patient and the primary physician as the team leader, HCP program believes all children with special health care needs should have an opportunity to receive services which facilitate their optimum development. Parents receive assistance in identifying available services to strengthen family life while coping with the unique needs of their children.
